Annual Diabetes Check

You will be invited for your annual diabetes review normally during your birthday month.

When you receive your invitation, please book a blood test appointment. On the day of your blood test, please bring an EARLY MORNING urine sample so we can check your ACR (Albumin-to-Creatinine Ratio).

Please use the POD in our waiting room to record your current weight, height and blood pressure. If you are unsure how to use the POD, our reception team will be happy to help.

Please also ensure that your diabetic foot check has been booked and completed before booking your follow-up appointment with the GP or Diabetes Nurse.

Your follow-up appointment should be booked approximately 2 weeks after your blood test. This allows enough time for all your test results to be received and reviewed before your appointment, ensuring your clinician has all the information needed to discuss your diabetes care with you.
